Objectives: To report the alignment and early results of the first ninety total knee replacements performed with the aid of a computerized infrared tracking system. Background: Last year we reported our early results of the first thirty total knee replacements performed with this navigation system. The system employs rigid infrared markers fixed to the bones along with digitized anatomic landmarks and a calculation of the femoral head center. Range of motion and alignment can be followed intraoperatively and adjustments can be made prior to insertion of final components. Design/Methods: The first ninety consecutive patients to undergo a computer-assisted total knee replacement, including the thirty from last year's study, were evaluated. Pre- and postoperative alignments were measured on four foot standing radiographs. Preoperative, 6 weeks, 3 months, 6 months, and 1 year ranges of motion were measured. Knee scores and function scores were calculated. Results: Ranges of motion, alignment, and knee and function scores obtained with computer-assisted total knee replacement are equal to or better than those obtained using standard knee systems. There was one broken distal femoral pin removed without incident. Our initial surgery times are twenty minutes longer. Eighty-one patients have postoperative standing radiographs for evaluation. Sixty-four patients are within one degree of perfect neutral alignment. Twelve patients had errors of 2 degrees. Four patients had 3 degree errors and one patient had 4 degree errors.
