Objectives: This study was undertaken to evaluate the assertion that the frontal plane alignment of the knee joint achieved during total knee arthroplasty surgery could be improved over present mechanical alignment systems by using a computer aided surgical system. Background: Total knee replacement surgery has been performed since the late 1960s. While this surgical procedure has dramatically improved the lives of millions of patients world-wide, the procedure still has room for significant improvements. Prosthetic component alignment, and the resultant knee joint alignment, is the most important factor influencing prosthetic component loosening and stability. A frontal plane (varus or valgus) mal-alignment of as little as 5 degrees or more is believed to lead to significantly increased component wear rates and may lead to an increase in component loosening (and therefore total knee arthroplasty failure) rates by as much as 50%, when compared to well aligned total knee arthroplasty components. New computer aided surgery, total knee arthroplasty (CAS-TKA) alignment systems are emerging in clinical practice, and these systems hold the promise of improved total knee component alignment accuracy, without the risk of fat or marrow embolism seen with the intramedullary alignment systems which are currently in widespread use.
