The use of minimized extracorporeal systems (MECC) is gaining clinical acceptance in coronary bypass grafting, because of the reduced inflammatory response that is responsible for organ dysfunctions and neurological disorders. Low priming volume influences the hemostasis and is associated with reduced blood product use. Aim of this study was the investigation of effects on organ function and neurological dysfunction. In a prospective, randomized study 90 patients underwent an elective coronary artery bypass grafting using either mini-extracorporeal circulation (MECC-group n=90) or conventional cardiopulmonary bypass (CPB-group, n=45) since March 2004. Reoperations and concomitant valve procedures were excluded. The mean age was 66+-9years in the CPB- group and 65+-8 years in the MECC- group. Blood levels of cytokines (IL6, IL 8, IL10, TNF alpha), complement fac-tors( C3,C4), myocardial markers (creatinkinase, Troponin T), S100B, haemoglobin and thrombocytes were serially measured at different time points (preoperatively, 0,1,6,12,24,48 hours after CPB).
