Analysis of surgical and epidemiological series as well as wait-and-scan series supports the proposal of the following classification of vestibular schwannomas based on the largest extrameatal diameter only: intrameatal tumors, small tumors (1-10 mm extrameatal size), medium tumors (ll-20mm extrameatal size), large tumors (21-40mm extrameatal size), and giant tumors (41mm extrameatal size or larger). We do, however, gladly support the proposal of the Kanzaki group to split large tumors into a group of moderately larger tumors of 21-30 mm extrameatal size and a group of large tumors of 31-40 mm extrameatal size, thus resulting in six size groups.
