Large and giant paraclinoidal ICA aneurysms often require different clipping techniques compared to those used in aneurysms of smaller size. Between 1989 and 2002, 54 patients with paraclinoidal carotid artery aneurysms have been treated. There were 19 large (11) and giant (8) aneurysms. There were 13 women and 6 men. The aneurysm was on me right side in 8 cases and on the left in 11. 8 patients presented with SAH, 9 patients had symptoms resulting from aneurysm mass, and in 2 patients the aneurysm was detected incidentally. The size of the aneurysm varied from 16 to 47 mm. Temporary artery occlusion has been employed during operation in 16 cases (84,2%). Multiple clips were required for satisfactory clipping in 14 cases (73,7%). 12 patients (63,2%) had a favorable outcome, 4 patients (21,1%) had an unfavorable outcome and 3 (15,8%) died.
