Ablative Stereotactic Surgery Improves Manual Performance Time in Parkinson's Disease

The objective of this study was to determine the influence of stereotactic ablative surgical interventions on the time required for the performance of manual tasks in patients with Parkinson's disease (PD). Therapeutic effects resulting from stereotactic lesions of the basal ganglia (pallidotomy and combined pallidothalamotomy) were analyzed in 28 consecutive patients with PD. The speed of performance of handwriting and drawing were assessed at four timepoints: before the operation, and 2 days, 3 months and 6 months postoperatively. An international standard scale (modified by Fahn) was employed to evaluate the effectiveness of the surgical interventions. According to their symptomatology, the patients were divided into 2 groups. Those in group A were characterized mainly by hypo- or akinesia, rigidity, "on/off fluctuation" and L-dopa-induced dyskinesia. In this group, pallidotomy was performed. The patients in group B exhibited not only the above symptoms and signs, but also severe resting tremor. They were treated by pallidothalamotomy. The time of performance of the manual tasks diminished significantly in all cases in both groups (Student's T test: p< 0.0001), i.e. significant improvements ensured in the speed of handwriting and drawing.
