Recently published papers tackling the outcome of patients operated on for herniated lumbar disc have reported that disabling chronic low back pain is often present in subjects who are under 40 at the time of surgery. In these young patients, especially in those that have been operated on for large dimension lumbar disc herniation, two common post-operative situations have been evidenced; frequent occurrence of micro-instability and/or an alteration in the Modic 2 signal (type) at MRI at 6-12 months. To prevent these conditions, a DIAM (Interspinous Stress-Breaker Device) was implanted in a sample of 50 such patients. At follow-up (12 to 14 months), no patient showed any micro-instability and less than 10% of those tested reported any disabling chronic low back pain.
