Background: Roux-en-Y gastric bypass (RYGB) is considered the best surgical procedure for treatment of morbid obesity. Methods: The Mini-open Roux-en-Y Gastric Bypass (MORYGB) surgical technique was developed and utilized in the treatment of 500 morbidly obese patients. Average BMI was 48.1 kg/m~2(35-80). Hospital stay was 3.2 days. Results: MORYGB were performed with low mortality (0.2%) and morbidity rates. Operating time was shorter - 68.8 min (45-89) and the size of the skin incision smaller compare to open (15-20cm) and laparoscopic (8.4-9 cm for 6 incisions) RYGB. Cost of the MORYGB was also significantly lower than laparoscopic RYGB. Weight loss was more than 70% of excess body weight at the 12 month and was maintained through 24-36 months. Conclusion: MORYGB is a safe, less invasive, clinically effective, relatively inexpensive treatment of morbid obesity and in majority of cases is preferable when compared to the open or laparoscopic RYGB.
