Male hypogonadism is a fairly common occurrence in men with type 2 diabetes mellitus. Testosterone deficiency is associated with reduced insulin sensitivity and several known cardiovascular risk factors, which include visceral obesity, hyper-glycaemia, dyslipidaemia, prothrombotic and pro-inflammatory states and endothelial dysfunction. Low testosterone is an independent risk factor for the development of the metabolic syndrome and type 2 diabetes. The commonest cause of death in a diabetic is cardiovascular disease and testosterone deficiency has been shown to lead to an increase in cardiovascular mortality. Early studies of testosterone replacement therapy in hypogonadal men with type 2 diabetes have reported improvements in insulin resistance, visceral adiposity, cholesterol and glycaemic control. Testosterone replacement also improves sexual function in a group of patients who are known to be difficult to treat.
