While patients with cardiac amyloidosis usually have extensive myocardial infiltration of amyloid protein, the precise mechanism by which fibril deposition impairs cardiac function remains unclear. Clinical data and patient observation have shown the degree of infiltration often lacks correlation with heart failure severity and patient mortality (1-3) These observations led to a physiologic experiment that demonstrated a toxic effect of the amyloidogenic light chains on cardiac tissue (4). A subsequent study showed light chains isolated from cardiac-involved AL amyloid patients increased cellular oxidant stress (ROS) compared to non-cardiac amyloid light chains and increased oxidative stress was prevented following the addition of an antioxidant (5). We investigated the use of antioxidants for treating patient with AL amyloidosis and cardiac involvement We hypothesized antioxidants would lower ROS generated by light chains and improve cardiac function Our primary endpoint was a change in Brain Natriuretic Peptide (BNP), which has been identified as a sensitive marker of cardiac involvement in patients with AL amyloidosis (6).
