INFLUENCE OF INFLAMMATION ON LIPID METABOLISM IN THE PATIENTS WITH RHEUMATOID ARTHRITIS

Several lines of evidence show that cardiovascular disease (CVD) is a leading cause of death in patients with rheumatoid arthritis (RA). Standardized mortality ratios for patients with RA dying from CVD are reported to be 1.13 to 5.25 (1, 2). Mortality is not only linked to RA, but strongly linked to RA severity. Total cholesterol (TC) in active RA is reported to be reduced and TC in patients with active RA is reported to be lower than in those with inactive RA (3). Investigators consistently observe reduced high-density lipoprotein cholesterol (HDL-C) levels and frequently find reduced low-density lipoprotein cholesterol (LDL-C) levels in RA patients (3). Thus, the contribution of abnormal lipid metabolism to atherosclerotic change in RA is not clear.Lipoprotein metabolism has been shown to be markedly altered during the inflammatory process in animal models (4) and in human inflammatory diseases (5). Serum amyloid A (SAA) is an apolipoprotein of HDL and a major acute phase protein that can increase to 1000 fold over normal level. During the acute phase response, SAA becomes the major apolipoprotein in HDL particles, and has been shown to change the metabolism and lipid composition of HDL-C (6). Thus SAA could be an important determinant of cholesterol metabolism in chronic inflammatory diseases like RA.The purpose of this study is to reveal influence of inflammation on lipid metabolism in patients with RA.
