Does C-Reactive Protein Orchestrate Inflammation in Acute Coronary Artery Disease?

In recent years, C-reactive protein (CRP), long associated with inflammation, has emerged as a clinical marker for acute coronary artery disease (CAD). Acute CAD is also associated with widespread neutrophil activation, suppressed neutrophil apoptosis and prolonged survival, nitrosative stress, enhanced IL-8 production and formation of platelet-neutrophil and neutrophil-neutrophil aggregates in the circulation. Here we present results that these events are interrelated and likely orchestrated by CRP. However, these actions are "hidden" and expressed only when the pentameric structure undergoes a conformational rearrangement, leading to formation of modified or monomeric CRP (mCRP). Our data suggest that mCRP rather than native CRP displays potent pro-inflammatory and pro-thrombotic properties, which can contribute to development of acute CAD.
