Severe pediatric allergic contact dermatitis (ACD) was observed in some children playing near household vegetation. The dermatitis was caused by urushiol oil in poison ivy. However, certain cases exhibited severe dermatitis. This dermatitis appeared to be intensified in a subgroup of children whose clothing had been exposed to a light household oil prior to exposure. We conjectured that the light oil was simply dissolving the urushiol rather than chemically reacting with it. We found that the behavior of the urushiol in light oil could be visually simulated using Rhodamine 6g dye from a red permanent marker. This dissolution of the urushiol, which is a 1, 2 di-hydroxy 3 alkyl benzene, reacted as an enhanced delivery system through clothing, resulting in unusually severe reactions after the exposure to poison ivy. This study was prompted by observations of infrequent but unusually severe reactions in certain accidentally exposed children. We surmised that preliminary exposure to light aliphatic oils like WD-40, perhaps from toys or sports equipment was dissolving and transporting the urushiol through the clothing to increase skin contact. This resulted in severe dermatitis. DART mass spectrometry was used to surface desorb the analytes. The mass spectra clearly showed the irritant was urushiol, and not a chemical derivative. Urushiols are actually a class of catechols that feature a benzene diol with hydrocarbon tails which may vary slightly between samples.
