Survival Data after EMR/ESD for Early Gastric Cancer

Early gastric cancer (EGC) is defined as gastric cancer in which tumor invasion is limited to the mucosa or submucosa, regardless of lymph node involvement.1 En-doscopic mucosal resection (EMR) has been widely accepted as a standard treatment for EGC with a negligible risk of lymph node metastasis, especially in Asian countries which have a high prevalence of gastric cancer.2,3 Standard indications for EMR of EGC include differentiated elevated cancers less than 2 cm in size and depressed cancers without ulceration less than 1 cm in size.4 However, these criteria have been considered as too strict and leading to unnecessary surgery.5 In Japan, expanded indications have been proposed to include other lesions with a negligible risk of lymph node metastasis, such as (1) mucosal cancer without ulcer findings irrespective of tumor size, (2) mucosal cancer with ulcer findings <30 mm, and (3) minute (<500 um from the muscularis mucosae) submucosal invasive cancers <30 mm in size.6 With the development of Endoscopic submucosal dissection (ESD), en-bloc resections of larger or ulcerated lesions have become possible, thus resulting in a more accurate histopathologic diagnosis and reduced local recurrence.5 Currently, ESD is rapidly gaining popularity as the first-line treatment for EGC in Korea and Japan.Patient survival data after endoscopic treatment for EGC, including long-term data after EMR and recent data after ESD, will be presented in this review.
