Rotator cuff tears, specifically of the supraspinatus tendon, are a common shoulder injury, resulting in significant pain and disability. With the widespread availability of arthroscopy and MRI, articular-surface tears of the supraspinatus tendon are believed to have a more significant role in shoulder pathology than previously recognized. Few studies have characterized the mechanical behavior of the rotator cuff and, in particular, the articular side is poorly understood. Furthermore, previous investigations into rotator cuff mechanics have only provided gross measures of surface tissue strain [1-3]. Thus, the purpose of this study was to compare intratendinous strain fields of intact shoulder specimens to the same specimens after creating an articular side partial-thickness rotator cuff tear. Based on the assumption that the insertion site is shielded from applied loads as the tendon wraps over the humeral head, we hypothesized that the tear would cause an increase in supraspinatus tendon strain only at glenohumeral joint angles greater than 45°.
