The commonest complication of orbital decompression in Graves' ophthalmopathy is still represented by the onset or worsening of diplopia, due either to the asymmetry consequent to the disease or the inadequacy of the decompression effected. The paper compares the results obtained with inferomedial approach with the results obtained with a medial and lateral approach. The aims of orbital decompression are maximal posterior displacement of the eye, to minimize the risk of the eye imbalance, to eliminate any compression of optic nerve, if it is present. Decompression can be carried out by removal of the intraconal fat (1) or the orbital walls (1,2,3,4,5). The commonest complication of such surgery is represented by the onset or worsening of diplopia, due either to the asymmetry consequent to the disease or the inadequacy of the decompression effected. The present report compares results obtained by approaching the orbital floor or not in two different groups of patients.
