PURPOSE: We evaluated the efficacy and change of contrast-enhanced ultrasonography after HIFU, and decided on the best timing for contrast-enhanced ultrasonography to evaluate the response of hepatocellular carcinoma with HIFU. MATERIALS AND METHODS: HIFU ablation was carried out without rib resection and the aid of transcatheter arterial chemoembolization (TACE) or percutaneous ethanol injection (PEI) in 11 patients with small hepatocellular carcinoma (< 3 lesions, < 3 cm in diameter). The HIFU system (Chongqing Haifu Tech) was used under ultrasound guidance. Assessment of contrast-enhanced (Sonazoid, CHA mode, MI 0.8) ultrasonography (Logic7, GE) was performed. RESULTS: HIFU treatments were successfully performed in all patients. After HIFU, treated lesions showed decreased vascular area with peripheral rim enhancement in the border of the ablated area at 30 seconds after the injection of Sonazoid in all cases. 7 days after the treatment, peripheral rim enhancement persisted in 6 of 11 lesions. The rim enhancement was completely resolved in all lesions after 3 months. During 6 months follow-up, the ablated lesions showed a gradual decrease in diameter. CONCLUSDION: The 7 days after HIFU is most appropriate and practical for the performance of contrast-enhanced ultrasonography to evaluate the therapeutic response.
