摘要

Internal herniation through a defect in the broad ligament of the uterus is rare. We herein report the caseof a 52-year-old woman with intestinal obstruction without a history of surgical treatment. An ovarian cyst wasalso present on the side of the broad-ligament defect. Ovarian torsion was diagnosed from Multi-detectorcomputed tomography (MDCT) findings, and emergency surgery was performed. Laparotomy showed a defecton the left side of the broad ligament without torsion of the ipsilateral ovarian cyst and intestinal herniationthrough the defect. Necrotic ileum was resected and followed by ovariectomy and resection of the impairedbroad ligament. Herniation could not be detected on MDCT because of the presence of the ovarian cyst in thepelvic cavity. A Medline search revealed dozens of cases of patients with internal herniation through a defect inthe broad ligament of the uterus, however, none of these patients had an ovarian cyst. Associations between adefect in the broad ligament of the uterus and ovarian cyst are unclear. However, chronic compression andinflammation by an ovarian cyst might cause weakness and defect of the broad ligament.
