Background & Aims: Urinary tract infection (UTI) is one of the most common bacterial infections in children. In up to 40% of the cases of pyelonephritis, renal scar develops and this process may occasionally lead to chronic renal insufficiency .Hence this prospective study was undertaken to find out PCT as a marker of severity of renal lesions in children with UTI. Methodology: A total of 100 children in the age group of 0-10 years with culture positive UTIs were included in the study group and followed over 2 1/2 year. Results: The prevalence of renal scarring after UTI is 31.2%, 70% of children had VUR on presentation. The prevalence of renal scarring secondary to VUR is 84%. Incidence of renal scar and VUR is significantly associated with PCT levels with P<0.001. Conclusions: Procalcitonin (PCT) levels well correlated with the severity of renal involvement at the time of diagnosis of febrile UTI and also with the risk of permanent scarring (Se:97% and Sp:87%) and VUR (Se:97% and SP:90%).Therefore PCT measurements could be a valuable tool for the early prediction of renal scarring and presence of VUR.
