Vascular dementia is the second most common cause of dementia, followingAlzheimer disease. The underlying mechanisms are largely unclear; multi-infarctdementia, borderline/watershed lesions, and severe hypoperfusion have beenimplicated. We described herein a case of acute vascular dementia and typicalacute watershed infarctions due to hypovolemia aggravaged cerebral ischemiaassociated with bilateral middle cerebral artery (MCA) stenosis. The infarctionsand acute dementia occurred almost at the same time, which, to our knowledge,has rarely been described before. The clinical symptoms and radiographicevidence indicated a possible causal link associating watershed infarction anddementia with hypovolemia aggravaged cerebral ischemia associated with MCAstenosis.
