Although pharmacists are rarely required to handle large loads, they work at a fast pace for long hours to deliver drugs to patients. This paper presents a Monte Carlo simulation model to estimate the rate of energy expenditure (Kcal/min) in pharmacies with high patient flows. The model considers various combinations of pharmacist's body mass and walking speed, pharmacy compact layout alternatives, number of drugs per script, and locations of drugs in the script. The scope of the study is limited to how walking and picking drugs during a picking route affect the energy expenditure rate of pharmacists. Replicates of 10,000 scripts, with a random number of drugs per script and random locations of drugs in each script, are used to estimate the average Kcal/min for each investigated combination of the study variables. Results illustrate the high impacts of body mass and walking speed, and the insignificant impacts of the gender of the pharmacist, the weight of the drug and the layout of the pharmacy on the Kcal/min. Compared to allowable Kcal/min; 2.6 for females and 4.2 for males, obtained results show a major early fatigue concern especially for female pharmacists under various combinations of body mass and speed of walk.
