摘要

About 15% of patients with diabetes mellitus develop a diabetic foot syndrome (DFS) during their lifetime. Complications caused by DFS are the main reason for hospital admissions and amputations [1] [2], The number of amputations in DFS patients is estimated at 40,000 per year and accounts for approximately 70% of all amputations performed in Germany. The total costs of treatment in Germany amount to approximately 2.5 billion euros per year. In order to reduce expenses, the DFS needs optimal care and treatment [3]. The data and information transfer is a decisive aspect in ensuring the quality of care and patient safety. The development and use of modem e-health applications is therefore a promising approach for the interdisciplinary treatment of DFS. The iFoot research project focuses on an optimized approach to DFS medical and nursing care. In 2005 the German Federal Association for Statutory Health Insurance Physicians (Kassenarztliche Bundesvereinigung, KBV) introduced a specification for the electronic Disease Management Programme (eDMP) for patients with Diabetes mellitus type 1 and type 2. The aim of the eDMP is the structured treatment of chronically ill patients with the help of defined care processes based on defined therapy goals and to improve communication and care between service providers and the inpatient and outpatient sectors of the German health care system [4].

EDMP的目的是结构化治疗慢性病患者患有基于定义的治疗目标的定义护理过程,并改善服务提供商与德国医疗保健系统的住院和门诊部门之间的沟通和关怀[4]。
