Nutritional deficiencies and mental and behavioral disorders rank among the leading causes of global disability. The goal of this dissertation research was to investigate the relation of iron status, as measured by an array of complete blood count and iron-specific measures, to both cognitive performance and depressive symptoms in older adults in the InCHIANTI study. We first carefully developed multiple methods based on the scientific literature to characterize iron status in order to assess if nontraditional methods that took age and inflammation into consideration would be more accurate in characterizing iron status than methods that used traditional cutoffs typically applied to younger populations. We then used our results from this assessment to look at the relation between iron status, as determined by the optimal method of iron status characterization that considers both age and inflammation, and cognitive performance and depressive symptoms in the InCHIANTI elderly as measured by the Mini-Mental State Examination (MMSE); the Trail Making Test (TMT); and the Center for Epidemiologic Studies Depression Scale (CES-D). We hypothesized that individuals with poorer iron status would perform worse on cognitive tests, and we hypothesized that individuals with poorer iron status would have more depressive symptoms compared to those with relatively better iron status.;In statistical analyses using data at baseline and then in separate analyses using longitudinal data from four time points, we found significant associations relating poorer iron status to poorer cognitive performance, controlling for a variety of demographic, nutritional, and medically related covariates. We also found significant associations between iron status and depressive symptoms, although some of these relations were in an unexpected direction.;In conclusion, using a novel combination of methods from scientifically based literature to optimally characterize iron status in elderly InCHIANTI individuals, we found evidence both cross-sectionally at baseline and longitudinally using data across multiple time points in favor of a relation between poor iron status and poor mental health outcomes. Acknowledging a possible relation between iron status and mental health may more efficiently advance research and practice to effectively seek solutions to improve the status of both.
