Validation of the RBANS Effort Index, Effort Scale, and Performance Validity Index in Three Clinical Samples.

In order to draw accurate conclusions from the test data, clinicians must be confident that their data are valid. If an examinee intentionally engages in suboptimal performance or effort, neuropsychological test data may be invalid. The inclusion of performance validity assessment in a test battery, along with clinical judgment, can help clinicians reliably identify test-takers who are malingering or who are putting forth inadequate effort. The current study examined the psychometric properties of three embedded performance validity measures for the Repeatable Battery for the Assessment of Neuropsychological Status (RBANS; Randolph, 1998). These embedded measures included the Effort Index (Silverberg, Wertheimer, & Fichtenberg, 2007), Effort Scale (Novitski, Steele, Karantzoulis, & Randolph, 2012), and the Performance Validity Index (PVI; Paulson, Horner, and Bachman, 2015). More specifically, the current study evaluated the concurrent validity of the RBANS ES, EI, and PVI using the "gold standard" criterion measure, the Test of Memory Malingering (TOMM). Participants included 43 TBI patients, 106 dementia/MCI patients, 18 healthy controls, and 10 invalid responders who were referred to a large, urban academic medical center for a comprehensive neuropsychological evaluation. The major results of this study indicated that all of the embedded RBANS measures had, at best, limited agreement with the TOMM. Results suggested that their routine use in clinical neuropsychological assessment cannot be supported at this time. However, subtle methodological issues may have contributed to these results. Recent research on performance validity testing with embedded RBANS scales was reviewed, and possible applications of the findings of this study are discussed.
