Over the last decade, the patient-centered medical home model of care has become a popular method of coordinated care. Leaders in the healthcare industry may look at a more efficient way of managing chronic illnesses. Gathering a list of best practices from New York, Vermont, New Hampshire, and Maryland PCMH programs can provide a better knowledge of effective care coordination and increase the management of chronic illness. Based on the research in this capstone, the author discussed the impact of the PCMH model on the health care industry and the health management of the U.S. population. This capstone examined some of the best practices of medical home programs in the states of New York, New Hampshire, Vermont, and Maryland. The number of people diagnosed with one or more chronic illnesses has increased significantly. The author looked at each program to gather information regarding best practices used to improve organizational communication, health information technology (HIT), and the patient experience. The data for this research project came from state documents as well as from the Patient-Centered Primary Care Collaborative website. National Commission of Quality Assurance PCMH requirements influenced the author's research. The best practices outlined involve the patient experience, the structure of the healthcare team, and the use of innovative HIT. The research allows leaders in the healthcare industry to look at how population health is managed as well as efficient methods to coordinated care.
