Differential effects of electroconvulsive therapy (ECT) on verbal and visuo-spatial functions in depressed patients.

Electroconvulsive therapy (ECT) is a safe and effective means of rapidly treating patients with severe depression. However, the side effects of ECT on verbal and visuo-spatial memory are of concern. Previous research presents conflicting evidence about the effects of bilateral (BL) and right unilateral (RUL) ECT on memory function. Some differences may be due to varying practices in the administration of ECT before the mid 1980s.;This study compared the differential effects of BL and RUL ECT on verbal and visuo-spatial memory tasks given to patients before and after they received treatment. Scores on the Hopkins Verbal Learning Test (HVLT), Benton Visual Retention Test (BVRT), Benton Supraspan, Trail Making Test, Neurobehavioral Cognitive Status Exam, Montgomery and Asberg Depression Rating Scale, and the Subjective Memory Questionnaire, were examined to determine how ECT affects various aspects of cognitive function. A review of medical records was also conducted. Repeated measures analysis of variance was used to evaluate the effect of RUL and BL ECT on pretest and posttest scores on the neuropsychological and psychological measures.;RUL and BL ECT patients showed similar responses to neuropsychological and psychological measures over time. Significant main effects for both groups were found on verbal tasks, with scores on the HVLT decreasing. Scores for tests of attention showed significant differences for both groups, with Benton Supraspan scores decreasing, and time to completion of Trails B and D increasing. Depression ratings were significantly lower after ECT, and perceived memory ability was significantly higher for both groups.;The similarity of performance for both groups would seem to indicate that there are no significant differences between the groups as a result of the treatment. However, observed differences between means in this study suggest the possibility of improved performance by the RUL ECT group on at least some cognitive measures, and this possibility should be explored in future research. It was suggested that the effects of RUL and BL ECT might lie in subtle, and less easily measured, aspects of cognition.
