目的 探讨炎症性肠病(IBD)并发贫血的临床特点及蔗糖铁治疗的疗效.方法 采用回顾性研究的方法,收集200例IBD患者,观察其并发贫血的临床特点,并进行蔗糖铁静脉滴注治疗和效果评估.结果 200例IBD患者中溃疡性结肠炎(UC)患者贫血发生率为38.99％;克罗恩病(CD)患者贫血发生率为65.85％,UC患者、CD患者的年龄、贫血发生率均差异有统计学意义(P＜0.05),UC患者与CD患者的性别构成差异无统计学意义(P＞0.05).多因素分析发现有吸烟史、有饮酒史、病变范围、疾病活动度分期均为UC、CD合并贫血的危险因素(P＜0.05).对贫血原因进一步研究发现叶酸、维生素B12缺乏也是造成贫血的重要原因.蔗糖铁静脉滴注治疗1周后缺铁性贫血(IDA)患者的血红蛋白(Hb)、铁蛋白、转铁蛋白饱和度、红细胞平均体积(MCV)中位数分别升高为98 g· L-1、35 mg·dL-1、28％、84Ⅱ,与治疗前相比均差异有统计学意义(P＜0.05),未发现明显不良反应.结论 IBD并发贫血发生率高,危险因素多,蔗糖铁对于IBD合并贫血具有较好的疗效.%Objective To explore the clinical characteristics of inflammatory bowel disease complicated with anemia and evaluate the therapeutic efficacy of iron sucrose.Methods By using the method of retrospective study and clinical experiment,the clinical characteristics diagnosed IBD patients to investigate the anemia,and to evaluate the therapeutic effect of iron sucrose.Results This study collected 200 IBD patients,including anemia in patients with UC incidence rate was 38.99％.The incidence of anemia in patients with CD 65.85％,UC and CD with age,the incidence of anemia were significantly different (P ＜ 0.05).The gender was not statistically significant different (P ＞ 0.05).Multivariate analysis showed that smoking history,drinking history,number of lesions and disease activity were risk factors of UC and CD with anemia (P ＜ 0.05),further study on the causes of anemia found an important reason of folic acid,vitamin B12 and etc.cause anemia.Compared with before treatment,iron sucrose treatment a week after treatment of IDA patients with HB,ferritin,transferrin saturation,median MCV were increased to 98 g · L-1,35 mg · dL-1,28％,84 fl,which was statistically significant (P ＜ 0.05),found no adverse reaction.Conclusion IBD with high incidence of anemia,more risk factors,iron sucrose for IBD with anemia has a better therapeutic effect.
