目的：比较Quadrant系统下sextant内固定术与传统后路腰椎椎体间融合（ posterior lumbar interbody fusion ，PLIF）开放手术治疗腰椎滑脱症的临床疗效及相关并发症。方法回顾性分析该院2008年1月-2012年1月收治的294例腰椎滑脱症患者，其中82例患者行Quadrant系统下sextant内固定治疗（微创组），212例行传统的PLIF治疗（传统组），分别记录两组患者的手术时间、术中出血量、术后引流量及输血量、术后卧床时间、疗效评定及各种并发症的发生率。结果微创组的手术切口、手术时间、术中失血，术后住院时间均明显少于传统组，两组差异有统计学意义（P＜0．05）；2组术后3个月ODI及VAS评分差异有统计学意义（P＜0．05），优良率也具有显著差异（P＜0．05）；术后微创组并发症发生率为3．66％，较传统组并发症发生率17．92％差异显著（P＜0．05）。结论 Quadrant通道下减压sextant内固定治疗腰椎滑脱症有较好的疗效，并且创伤小、并发症发生率低，是临床上值得推广的手术方式。%Objective To compare the clinical efficacy and related complication of Quadrant decompression combined with sextant fixa -tion and posterior lumbar interbody fusion ( PLIF) in the treatment of lumbar olisthe .Methods Two hundred and ninety-four cases of lumbar olisthe disease patients admitted to hospital from January 2008 to January 2012 were retrospectively analyzed ,including minimal-ly invasive group 82 cases (operated under Quadrant system ) and traditional surgery group 212 cases.The operation duration,intraoper-ative blood loss ,postoperative flow ,the amount of blood transfusion ,postoperative bed time ,curative effect evaluation and the incidence of various complications were recorded respectively .Results The incision,operation time,intraoperative blood loss,postoperative hos-pital stay of minimally invasive group were significantly less than the traditional group ( P <0 .05 );The ODI and VAS scores 3 months after operation and the effective rate were statistically different (P <0.05).The incidence of postoperative complications of minimally invasive group (3.66%) was significantly different from the incidence of complications in the traditional group (17.92%) ( P<0.05).Conclusions The Quadrant decompression combined with sextant fixation for lumbar olisthe has a good curative effect with ad -vantages of small trauma ,and low complication rate ,which is worthy of clinical application .
