基于HIS信息资源的医疗费用构成及趋势分析

摘要

Objective: To analyze the medical expenses structure and tendency among inpatients, and to master the constitution ofrninpatients' medical expenses under different ways of payment through HIS. Methods: To analyze and compare medical expense ofrninpatients in a hospital in Shanghai through VSV, DSV and contribution rate of structure variation. Results: Medical expense per timernof free-paid payment is the highest among the three modes of payment, self-paid payment is lowest. The drug expenses constituternis the largest among the three modes of payment, and the bed expense constitute is larger in self-paid mode. The degree of structurernvariation per year of self-paid mode is the largest, and the main reason is the constitution of drug expense change and decrease of bedrnexpense year by year. Conclusion: Data mine could be done based on HIS. The development of social insurance system progressedrnsmoothly and the medical expense of basic medical insured has been controlled on a certain extent. The health system reform includingrnfree-paid and self-paid mode should be improved.%目的:利用HIS系统信息,了解不同支付方式下住院患者的医疗费用构成及变动趋势,为控制医疗费用过快上涨提供方法学基础.方法:运用结构变动值、结构变动度和结构变动贡献率等评价指标,以上海市某综合性三甲医院2007-2010年住院患者费用的变动情况为例进行分析.结果:公费医疗的次均住院费用最高,医保次之,最后是自费.各种支付方式下的药品费用均占较大比重,自费医疗中床位费构成较大.自费患者住院费用构成2007-2010年年均变动度最大,主要是床位费逐年降低和药费构成波动引起.结论:可以通过HIS系统进行数据挖掘.社会医疗保险体系正逐步步入正轨,医保患者住院费用得到了一定控制；药品费用仍然是住院患者医疗费用的主要构成,在公费患者中尤为显著；自费患者床位费较高.今后对公费医疗总费用和自费患者床位费应有一定控制,结合医疗保险制度,进一步完善医药卫生体制配套措施的改革.
