256层CT前瞻性心电门控冠状动脉成像研究

         

摘要

Objective: To compare the image quality and patient radiation dose between prospective ECG-triggered and retrospective ECG-gated CT coronary angiography (CTCA) by using 256-slice CT. Methods: A total of 200 patients were enrolled in this study. All patients with an average heart rate (HR) <70 bpm, 100 patients were scanned by using prospective ECG-triggered and 100 patients by retrospective ECG-gated with 256-slice CT, The image quality and the radiation dose were compared in two groups. Results: of 800 coronary artery, excellent or good image quality was achieved in 96.2°/o(385/400) in group A, 97.3°/o(389/400) in group B, with no significant difference for A vs B. The average radiation dose in group A [(2.9±0.5)mSv] reduced 73% comparing with that in group B[(10.2±2.4)mSv]. Conclusion: Using prospective ECG-triggered to perform CCTA with 256-slice CT could keep the image quality and reduce radiation dose significantly.%目的:评价256层CT前瞻性心电门控冠状动脉成像的图像质量与辐射剂量,探讨前门控技术的临床应用价值.方法:将200例患者分为前瞻性心电门控检查组(A组)与回顾性检查组(B组)2组,每组患者100例纳入研究.2组分别行前瞻性心电门控与回顾性心电门控扫描,扫描前均控制心率≤70次/min,分析组间冠状动脉图像质量并比较组间的辐射剂量.结果:在2组800支冠状动脉血管中,A组图像质量优秀或良好者达到96.2%(385/400),B组为97.3%(389/400),A组与B组冠脉图像质量无明显差异;其平均辐射剂量A组为(2.9±0.5)mSv,B组为(10.2±2.4)mSv,A组与B组相比平均辐射剂量减少73%.结论:256层CT前瞻性心电门控技术可获得满足诊断需要的图像质量,且显著降低了辐射剂量.
