目的 探讨高频彩超在肱二头肌长头肌腱炎诊断中的应用价值.方法 选择2016年3月～2018年2月在扬州市中医院骨伤科门诊诊断为肩周炎,经高频彩超诊断为肱二头肌长头肌腱炎的患者60例.使用高频彩超先后取横切面和纵切面分别观察患侧及健侧肱二头肌长头肌腱的厚度、内部回声、钙化灶及血流分布声像图.将60例患者分为急性期组及慢性期组,比较两组患者患侧与健侧肌腱的声像图表现.结果 在急性期36例患者中,患侧的肌腱厚度、回声减低、血流增多超声表现指标高于健侧,差异有统计学意义(P＜0.05);在慢性期24例患者中,患侧的回声杂乱、内部钙化点、血流增多超声表现指标高于健侧,差异有统计学意义(P＜0.05).结论 高频彩超在肱二头肌长头肌腱炎的定位、定性诊断方面具有明确、快速、廉价和重复性高等特点,可作为肱二头肌长头肌腱炎的首选检查方法.%Objective To evaluate the diagnostic value of high frequency color ultrasonography in tendinitis of caput longum musculi bicipitis brachii.Methods A total of 60 patients who were diagnosed with scapulohumeral periarthritis and long head bicep tendinitis by high frequency color ultrasonography at Orthopedic Clinic of Yangzhou Traditional Chinese Medicine Hospital from March 2016 to February 2018 were selected.Thickness,internal echo,calcification and blood flow distribution sonogram of both affected and unaffected long head bicep tendons were observed from cross section and longitudinal section.In order to compare differences between affected and unaffected long head bicep tendons,those 60 patients were divided into the acute period and the chronic period.Results Of the 36 acute patients,the thickness of tendon,echo reduction,and blood flow increase of the affected side were higher than those of the unaffected side,the differences were statistically significant (P ＜ 0.05).Of the 24 chronic period patients,echo clutter,internal calcification,and blood flow increase of the affected side were higher than those of the unaffected side,the differences were statistically significant (P ＜ 0.05).Conclusion High frequency color ultrasonography is clear,fast,cheap and repeatable in positioning and qualitatively diagnosing tendinitis of caput longum musculi bicipitis brachii.Therefore,it can be regarded as the preferred method of examination.
