目的：调查并分析陕西省宝鸡市中心医院(以下简称“我院”)开展抗菌药物临床应用专项整治活动的成效与不足，为进一步深化抗菌药物专项整治，规范抗菌药物的临床应用提出建议。方法随机抽取2010年10~12月(专项整治前)和2013年10~12月(专项整治后)我院Ⅰ类切口手术(甲状腺、乳腺、腹外疝手术)病历各200份，分析整治前后抗菌药物使用率和用药情况。结果通过专项整治，我院Ⅰ类切口手术抗菌药物预防使用率由98.5%降为7.0%，药物选择、给药时机、溶媒选择、单次剂量的合理率分别由47.72%、10.66%、52.38%、41.12%升至78.57%、57.14%、100.00%、78.57%，差异均有统计学意义(P<0.05)，但仍存在用药疗程合理率无明显提高等问题。结论专项整治活动有助于促进抗菌药物的合理使用，建立抗菌药物临床应用管理长效工作机制是必要的。%Objective To investigate and analysis the effect and inadequacy of the special regulation of the using of an-tibiotics in Baoji Central Hospital of Shaanxi Province (“our hospital” for short), offer a proposal for constantly deep-ened to promote the rectification and rational using of antibiotics. Methods 200 cases of type I incision operation (thy-roid, breast, abdominal external hernia) from October to December 2010 (before rectification) and 200 cases with the same operation from October to December 2013 (after rectification) in our hospital were randomly selected; rationality of antibiotics usage before and after rectification was contrastive analyzed. Results Through the special rectification, preventive antibacterial rate of type I incision operation reduced from 98.5%to 7.0%in our hospital, reasonable rate of selection of drug, medication timing, selection of solvent, a single dose increased from 47.72%, 10.66%, 52.38%, 41.12%to 78.57%, 57.14%, 100.00%, 78.57%, the differences were statistically significant (P< 0.05), but there were still some problems, such as reasonable rate no significant increase in course of prophylactic use. Conclusion The spe-cial rectification can contribute to promoting the rational use of antimicrobial, it is necessary to set up the effective and long-term working mechanism of antibiotics'management.
