Objective To investigate the atypical CT performances of small hepatocellular carcinoma (SHCC),and improve its diagnostic accuracy.Methods The rare CT findings of 19 cases of SHCC (24 lesions) confirmed by operatively and pathology were analyzed retrospectively,and reviewed the literatures.Results 24 lesions were found in 19 patients,of whom,4 cases with multicentric occurrence,the lesion morphology was patchy in 1 case; there were punctate fat density in lesions in 4 cases of CT scan; the lesion rupture with hemorrhage in 1 case; the lesions became isodensity in the portal venous phase and hypodense in the equilibrium phase in 3 cases; the lesions became isodensity in the portal venous phase and isodense in the equilibrium phase in 2 cases; 3 cases were all hypodense in all of the phases.Conclusion The rare CT features of SHCC are varied,the blood supply of lesions,liver physiological and pathological states and CT scanning technology are the causes of these findings.%目的 探讨小肝细胞癌的少见CT表现及原因,以提高小肝细胞癌的诊断正确率.方法 回顾性分析经手术及病理证实的19例小肝细胞癌共24个癌灶的少见CT表现,并结合文献进行复习.结果 19例小肝细胞癌共发现24个癌灶,其少见CT表现有:4例患者病灶多中心起源；1例病灶形态表现为斑片状;4例CT平扫内见点状脂肪密度；1例病灶破裂伴出血；3例门脉期呈等密度,平衡期呈低密度;2例门脉期呈等密度,平衡期仍呈等密度；3例癌灶始终无强化.结论 小肝细胞癌的少见CT表现多种多样,病变的血供情况、肝脏生理、病理状态和CT扫描技术等是其产生的常见原因.
