神经元特异性烯醇酶和凝血功能在评价脑外伤预后中的意义

         

摘要

Objective To investigate the significance of neuron-specific enolase and coagulation function in evaluation of the prognosis in patients with traumatic brain injury. Methods 66 patients with traumatic brain injury, from January 2009 to December 2011, in accordance with the severity of the traumatic brain injury, were divided into 3 groups: mild group (22 cases), medium group (26 cases), severe group (18 cases) and 30 healthy persons at the same period also were observed. The serum levels of neuron-specific enolase (NSE), prothrombin time (PT), activated partial thromboplastin time (APTT) and fibrinogen (Fbg) were detected in each groups. Results The level of NSE, PT, APTT and FBG increased from the control group, mild group, medium group and to severe group in turns, the differences were statistically significant (P< 0.01). In survival group the levels of NSE, PT, APTT and Fbg were significantly lower compared with the live group, the differences were statistically significant (P < 0.01). Conclusion The serum levels of NSE, the PT and APTT and Fbg in patients with traumatic brain injury play an important role in judging the severity of patient's condition, they are important for the prognosis of traumatic brain injury.%目的 探讨神经元特异性烯醇酶和凝血功能在评价脑外伤预后中的意义.方法 选取2009年1月~2011年12月在我院住院治疗的脑外伤患者66例为观察组,同期健康体检者30例为对照组,按照脑外伤的严重程度将观察组分为轻型组22例,中型组26例,重型组18例.检测并比较血清中神经元特异性烯醇酶(NSE),凝血酶原时间(PT),活化部分凝血活酶时间(APTT)和纤维蛋白原定量(Fbg)水平.结果 血清NSE、PT、APTT和Fbg水平在对照组、轻型组、中型组和重型组中依次升高,差异均有高度统计学意义(均P < 0.01).生存组血清NSE、PT、APTT和Fbg水平较死亡组明显降低,差异均有高度统计学意义(均P < 0.01).结论 脑外伤患者血清NSE、PT、APTT和Fbg水平与病情有关,对于判断预后有着重要意义.
