地龙野芭蕉改善老年高血压伴腔隙性脑梗死患者纤溶内皮功能的临床观察

摘要

目的：探讨地龙野芭蕉治疗老年性高血压伴腔隙性脑梗死（PH+LI）的疗效。方法对收治的50例老年 PH+LI 随机分为治疗组和对照组，每组25例，对照组用常规疗法，治疗组在常规疗法的基础上用地龙野芭蕉处方治疗30d，于治疗前后比较两组患者症状体征、纤溶功能（tPA、PAI）和血管内皮功能（ET、NO、PGI2）指标的改善情况，并与20例健康体检者比较。结果治疗组痊愈24例，显效1例，治愈率为96.0％；对照组痊愈23例，显效2例，治愈率为92.0％。两组疗效比较，差异无统计学意义（χ2=0.355，P=0.522）；两组患者 tPA、PAI、ET、NO、PGI2水平治疗前均有异常改变（P ＜0.01），治疗组治疗后缓解至健康组水平（P＞0.05），对照组只有 NO 水平治疗后缓解至健康组水平（P ＞0.05），tPA、PAI、ET、PGI2水平治疗后有所缓解，但与健康组比较差异仍有统计学意义（P ＜0.01）。结论地龙野芭蕉对改善老年 PH+LI 患者的血栓前状态和血管内皮受损有明确的效果，对预防脑梗死再发或许有积极的作用。%Objective To explore the curative effect of earthworm wild banana in treatment of elderly hypertensive patients with lacunar cerebral infarction (PH+LI). Methods 50 cases of elder patients with PH+LI cured in our hospital were randomly divided into treatment group and control group with 25 cases in each. Patients in control group were treated with conventional therapy, and patients in treatment group were treated with earthworm wild banana for 30 days on the basis of conventional therapy. The improvement situation of alleviate symptoms and signs, fibrinolytic function (tPA and PAI) and vascular endothelial function, (ET, NO and PGI2) of patients in the two groups before and after the treatment were compared. Results There were 24 cases recovery, 1 case excellence in treatment group, and the cure rate is 96.0%. There were 23 cases recovery, 2 case excellences in treatment group, and the cure rate is 92.0%. There was no statistically significant difference between the two groups (χ2=0.355, P=0.355). The tPA, PAI, ET, NO and PGI2 levels before the treatment of patients in the two groups had abnormal changes (P < 0.01). Treatment group after treatment were ease to health level (P > 0.05), but control group after treatment only the NO level change and healthy group (P > 0.05). The tPA and PAI, ET, PGI2 levels after treatment were eased, but there were still statistical significance on the differences compared with healthy group (P < 0.01). Conclusion Earthworm wild banana in treatment of the pre thrombotic state and vascular endothelial damage of elderly patients with PH+LI has clear effect. And it had a positive effect to prevent cerebral infarction recurrence.
