目的：探究经纤维胆道镜对复杂性胆道结石的治疗疗效、术后疼痛的影响及预防复发的效果，为临床研究提供相关参考价值。方法回顾性分析2009年12月～2013年12月我院内因胆道结石入院接受治疗的100例患者，根据治疗方法不同分为研究组和对照组，其中研究组患者胆道结石治疗方式选择纤维弹道镜，对照组患者手术采用常规胆道探查术。对比两组患者手术状况以及术后并发症发病率。结果研究组出血量较少，排气时间短，住院时间较短，上述指标均优于对照组患者，差异有统计学意义（ P＜0.05）。术后，研究组疼痛发生率为12%，明显低于对照组的28%，差异有统计学意义（P＜0.05）；研究组疼痛持续时间和VAS评分均明显低于对照组（P＜0.05）。两组感染发生率无统计学意义（P＞0.05），而组间结石残留率、胆漏发生率和复发率比较，差异有统计学意义（P＜0.05）。结论复杂性胆道结石患者采用纤维胆道镜进行治疗，手术中出血量较少、术后排气较快、住院时间较短，且在术后并发症方面，残石以及复发率均较低，疗效可靠。%Objective To study the therapy effect,influence of postoperative pain,effect of prevention recurrence of fiber cholangioscopy in the treatment of complex biliary calculi,to provide correlative reference value for clinical study.Methods 100 patients with biliary calculi,who were treated in our hospital from December 2009 to December 2013,were retrospectively anayzed,and were divided into study group which was treated with fiber cholangioscopy and control group which was treated with routine biliary exploration,according to different treatment methods.To contrast the situation of operation and the incidence of postoperative complications of patients between the two groups. ResultsThe indexes of less blood loss,shorter exhaust time,shorter hospitalization time in study group were better than which in control group,the differences were statically significant(P<0.05).The incidence of postoperative pain in study group(12%)was obviously lower than which in control group(28%),the pain duration,VAS score in study group was obviously lower than which in control group,the differences was statically significant(P<0.05).The incidence of infection between the two groups had no statistical significance(P>0.05),while the residual rate of stone,the incidence and the recurrence rate of bile leakage between the two groups had statically significant(P<0.05).Conclusion Fiber cholangioscopy in the treatment of complex biliary calculi has less intraoperative blood loss,quicker postoperative exhaust,and shorter hospitalization time,has lower incidence of postoperative complications,residual rate of stone, and recurrence rate,has reliable effect.
