β-catenin、P53和E-cadherin在食管鳞癌组织中的表达及临床病理研究

         

摘要

目的:探讨β-连环蛋白(β-cat)、P53和上皮钙黏素(E-cad)在食管鳞癌组织中的表达及与临床病理的关系。方法2005年5月~2014年8月,采用免疫组织化学Envision二步法检测226例食管鳞癌中β-cat、P53和E-cad的表达,分析三者的表达与不同临床病理特征的关系,对有随访资料的数据运用COX比例风险模型分析多因素与患者预后的关系。结果食管鳞癌组织中β-cat、P53和E-cad的阳性率分别为34.51%、70.35%和67.26%。食管鳞癌中β-cat、P53和E-cad阳性表达与患者的性别、年龄、肿瘤大小及分化程度无相关性(P>0.05),而与肿瘤浸润深度、TNM分期和淋巴结转移有关(P<0.05)。食管鳞癌中E-cad表达与P53、β-cat表达均呈负相关(r=-0.685、-0.637,P<0.05),E-cad表达和患者生存率呈正相关(P<0.05),β-cat、P53表达与患者生存率呈负相关(P<0.05)。结论E-cad可以抑制食管鳞癌的浸润、转移,与P53、β-cat的表达具有相关性,提示三者在食管鳞癌的发生、发展过程中共同发挥作用,联合检测三者可为评估食管鳞癌的预后及指导靶向治疗提供重要参考依据。%Objective To study the expression of β-catenin (β-cat),P53 and E-cadherin (E-cad) in esophageal squa-mous cell carcinoma issue the relationship between above-mentioned indicators and clinical pathology. Methods From May 2005 to August 2008,Immunohistochemisty of Envision two step method was applied to detect the expression of β-cat,P53 and E-cad in 226 cases with esophageal squamous cell carcinoma,and relationship between the expression of the three and different clinical pathological characteristics was analyzed.The relationship between multiple factors and prognosis of patients was analyzed using COX proportional hazard regression model for data having follow-up data. Results The positive rate of β-cat,P53 and E-cad in esophageal squamous cell carcinoma issue was 34.51%,70.35%and 67.26% respectively.The expression of β-cat,P53 and E-cad in esophageal squamous cell carcinoma was not cor-related with sex,age,tumor size and differentiation degree (P>0.05),and was associated with tumor invasion depth,TNM stage and lymph node metastasis (P<0.05).The expression of E-cad in esophageal squamous cell carcinoma was nega-tively correlated with the expression of P53 andβ-cat (r=-0.685,-0.637,P<0.05),the expression of E-cad was positively correlated with the survival rate of patients (P<0.05),the expression of β-cat,P53 was negatively correlated with the sur-vival rate of patients (P<0.05). Conclusion E-cad can inhibit the invasion and metastasis of esophageal squamous cell carcinoma,and associated with the expression of P53 and β-cat.This tips they play a role commonly in occurrence and development of esophageal squamous cell carcinoma.Detection of E-cad,P53 andβ-cat can provide important reference for evaluating the prognosis of gastric cancer and guiding targeted therapy.
