Objective To investgate the efficacy and safety of doxofylline and aminophylline for acute exacerbation chronic obstructive pulmonary disease ( AECOPD ) . Methods 60 patients with AECOPD from June 2013 to March 2015 in the ICU were selected and ran-domly divided into the treatment group and the control group. The treatment group was given doxofylline treatment, the control group was treated with aminophylline. The efficacy, the blood Qi and adverse reaction of the two groups were comparedbefore and after treat-ment. Results The total effective rate of the treatment group was 90. 00%, which was significantly higher than 73. 33% of the control group ( P < 0. 05 );compared with before treatment, the levels of pH, PaCO2, PaO2, SaO2 in the two groups were significantly improved ( P < 0. 05 ) . Compared with the control group treatment group after treatment, the improvement of the PaCO2, PaO2 in the treatment was more obvious ( P < 0. 05 );the incidence rate of cardiac arrhythmias, palpitations, chest tightness, nausea, vomiting, insomnia, headache inci-dence were decreased significantly in the treatment group ( P < 0. 05 ) . Conclusion Doxofylline has a significant efficacy in acute exac-erbation chronic obstructive pulmonary disease, can significantly improve the relevant indicators of blood gas analysis, has high safety and is worthy of clinical promotion.%目的：探讨多索茶碱和氨茶碱分别用于急性加重期慢性阻塞性肺疾病（AECOPD）治疗的临床疗效及其安全性。方法选取2013年6月至2015年3月医院重症监护室（ICU）收治的AECOPD患者60例，随机分为治疗组与对照组，治疗组患者给予多索茶碱治疗，对照组患者给予氨茶碱治疗，并对比治疗组与对照组患者的疗效、治疗前后血气分析及并发症情况。结果治疗组总有效率为90.00%，对照组为73.33%，与对照组对比，治疗组总有效率显著上升（ P<0.05）；与治疗前对比，两组患者的pH、二氧化碳分压（PaCO2)、氧分压（PaO2)、血氧饱和度（SaO2）均明显改善，差异有统计学意义（ P<0.05）；治疗后与对照组对比，治疗组患者的PaCO2及PaO2水平改善程度更明显，差异有统计学意义（ P<0.05）；治疗组患者心律失常、心悸、胸闷、恶心呕吐、失眠头痛的发生率显著降低，差异有统计学意义（ P<0.05）。结论多索茶碱应用于AECOPD具有显著疗效，能显著改善患者血气分析相关指标，安全性高，值得临床推广。
