视网膜母细胞瘤化疗前后血清乳酸脱氢酶水平的变化及意义

摘要

目的 探讨视网膜母细胞瘤化疗前后血清乳酸脱氢酶(LDH)的变化及其对化疗疗效的评价.方法 回顾性分析31例接受CEV方案(卡铂+依托泊苷+长春新碱)化疗的视网膜母细胞瘤患者,测定化疗前后血清LDH水平,行眼底检查对肿瘤分期并记录化疗前后的肿瘤变化.分析LDH、性别、年龄、眼别及肿瘤分期与化疗疗效的关系.结果 化疗前血清LDH为255.7U/L,化疗后为229.1U/L,差异有统计学意义(t=2.748,P=0.01).化疗后血清LDH水平降低者的疗效高于LDH水平升高者,差异有统计学意义(x2=6.339,P=0.042)；分期为D期以下者的疗效高于分期为D期及以上者,差异有统计学意义(x2=4.895,P=0.041)；而年龄、眼别及性别对化疗疗效无影响.结论 视网膜母细胞癌化疗前后血清LDH变化及肿瘤分期是影响化疗疗效的重要因素,可以作为疗效判断的参考评价指标.%Objective To discuss the alterations of serum lactate dehydrogenase( LDH) before and after chemotherapy and its prognostic value of the chemotherapy effect. Methods Thirty-one patients with retinoblastoma were enrolled in the study, and serum LDH was detected by Olympus 2700 automatic biochemical analyzer. Through RetCam Ⅱ fundus examination system, the stage and change of the tumor was recorded. The relationship between age, sex, eye involved, tumor stage, change of serum LDH and chemotherapy effect was analyzed by SPSS 13. 0 statistical analysis software. Results Serum LDH was 255. 7U/L before chemotherapy and 229. 1U/L after chemotherapy, the change was statistically different( t = 2. 748 ,P = 0. 01) . Patients whose serum LDH decreased after chemotherapy had better chemotherapy effect than those with increased serum LDH, showing significant differences (%2 = 6. 339, P = 0. 042). Patients whose tumor stage below D stage had better chemotherapy effect than those above D stage, showing significant differences^2 = 4. 895,P =0. 041). There was no relevance between age, sex, eye involved and chemotherapy effect. Conclusion The change of serum LDH level and tumor stage are factors that affect chemotherapy effect. It can provide an objective evaluation to determine the efficacy of chemotherapy.
