目的：探讨采用鼻内镜下手术治疗复发性Ⅲ型慢性鼻-鼻窦炎鼻息肉的临床效果。方法：选取笔者所在医院2012年1月-2014年3月收治的150例复发性Ⅲ型慢性鼻-鼻窦炎鼻息肉患者作为研究对象，按照随机数字表法分为对照组与治疗组，每组75例，对照组采用常规鼻息肉根治术，治疗组采用鼻内镜下手术，对两组患者术中出血量、治疗效果、并发症等进行分析比较。结果：治疗组术中出血量为(25.4±4.8)ml，明显少于对照组的(46.2±9.7)ml；并发症主要为炎性增生、脓性分泌物多、术腔粘连和窦口狭窄或闭锁，治疗组的并发症发生率为24.0%，明显少于对照组的37.3%；术后随访6个月~1.5年，治疗组的总有效率为74.7%，明显高于对照组的41.3%，两组患者术中出血量、总有效率和并发症的发生率比较差异均有统计学意义(P<0.05)。结论：采用鼻内镜下手术治疗复发性Ⅲ型慢性鼻-鼻窦炎鼻息肉可减少术中出血量，提高总有效率和降低并发症，可在临床广泛推广应用。%Objective:To investigate the clinical effect of endoscopic treatment of recurrent type III chronic nasal sinusitis and nasal polyps. Method:150 cases of recurrence of type III chronic nasal sinusitis nasal polyps patients were selected as the research object from January 2012 to March 2014 in our hospital,and they were randomly divided into the control group and the treatment group,75 cases in each group,the control group was treated with routine nasal polyps treated with radical mastectomy,while the treatment group was given nasal endoscopic surgery,the blood loss,therapeutic effect and complications were analyzed of the two groups.Result:The intraoperative bleeding of the treatment group was (25.4+4.8)ml, significantly less than that of the control group(46.2±9.7)ml,the complication were mainly inflammatory hyperplasia,purulent secretions,nasal cavity conglutination and sinus ostium stenosis or atresia and complications,the complication incidence rate of the treatment group was 24.0%,significantly less than that of the control group(37.3%),postoperative follow-up of 6 months to 1.5 years,the total efficiency of the treatment group was 74.7%, significantly higher than that of the control group(41.3%),the differences of the amount of bleeding,the total efficiency and the complication incidence rate were statistically significant in the operation in the two groups(P<0.05).Conclusion:Using nasal endoscopic treatment of recurrent type of chronic nasal sinusitis and nasal polyps can reduce the amount of bleeding,improve the total efficiency and reduce complications,and it is worth to be applied in clinical.
