目的：了解尿液分析试纸条目测法在家庭医学诊疗中的应用价值。方法选取2015年10—12月香港大学深圳医院家庭医学科门诊收集的203例患者的尿液样本，同时采集所有患者新鲜中段尿样本2份。1份尿液样本采用尿液分析试纸条目测法检测，另1份尿液样本送至检验科进行尿常规检查。以检验科尿常规检查结果作为金标准，计算尿液分析试纸条目测法检测尿白细胞、隐血、蛋白质、亚硝酸盐的灵敏度、特异度、阳性预测值、阴性预测值。结果尿液分析试纸条目测法检测尿白细胞、隐血、蛋白质、亚硝酸盐的灵敏度分别为57.4﹪、90.9﹪、61.0﹪、100.0﹪，特异度分别为70.5﹪、90.1﹪、43.9﹪、100.0﹪，阳性预测值分别为37.0﹪、34.5﹪、43.9﹪、100.0﹪，阴性预测值分别为84.6﹪、99.4﹪、81.0﹪、100.0﹪。尿液分析试纸条目测法检测尿白细胞、隐血、蛋白质、亚硝酸盐的阳性率分别为35.96﹪、14.29﹪、40.39﹪、2.46﹪。结论尿液分析试纸条目测法检测尿常规的阴性预测值高，可以作为一种可靠的筛检试验方法，为全科医生临床决策提供参考，且成本低，在家庭医学诊疗中具有较好的应用价值。%Objective To explore the value of visually read urine dipstick test in family medicine practice. Methods Two hundred and three patients were recruited from Department of Family Medicine,the University of Hong Kong ―Shenzhen Hospital from October to December 2015. Two fresh midstream urine samples were collected from these patients at the same time. One sample was sent to laboratory for machine urinalysis,the other one was sent for urine dipstick test and read visually. The laboratory results were set as the gold standard. The sensitivity,specificity,positive and negative predictive value of visually read urine dipstick test in the measurement of white blood cell,occult blood,protein and nitrite in urine were evaluated. Results The sensitivity of visually read urine dipstick test for measuring white blood cell,occult blood,protein and nitrite in urine was 57. 4﹪,90. 9﹪,61. 0﹪,100. 0﹪,respectively,the specificity was 70. 5﹪,90. 1﹪,43. 9﹪,100. 0﹪,separately, the positive predictive value was 37. 0﹪,34. 5﹪,43. 9﹪,100. 0﹪,respectively,and the negative predictive value was 84. 6﹪,99. 4﹪,81. 0﹪,100﹪,respectively. The positive rate of white blood cell,occult blood,protein and nitrite in urine measured by visually read urine dipstick test was 35. 96﹪, 14. 29﹪, 40. 39﹪, 2. 46﹪, respectively. Conclusion The visually read urine dipstick test has high value in detecting the negative predictive value of white blood cell,occult blood,protein and nitrite in urine, so its findings can be used as reliable reference information for family physicians. Moreover, for the advantages of low cost,it has good application prospect in family medicine practice.
