老年结核病患者自我健康管理评定量表的信效度研究

摘要

目的 评价老年结核病患者自我健康管理评定量表的信效度.方法 于2015年9月—2016年9月在武汉市武昌、汉口、汉阳各随机选取1个社区,以该3个社区中的360例老年(≥60岁)肺结核病患者为研究对象.以本课题组前期构建的老年结核病患者社会干预效果评定量表为基础,采用其中老年结核病患者自我健康管理评定部分对纳入患者进行调查,包括知识(BK)、态度(BA)、行为(BP)3个分量表.首次发放问卷360份,有效回收344例,问卷有效回收率为95.6％;2周后,采用随机数字表法抽取50例进行重测,问卷均有效回收,问卷有效回收率为100.0％.通过计算量表的Cronbach′sα系数、分半信度、重测信度、反应度评价其信度,通过分析量表的内容效度、结构效度、区分效度评价其效度.结果 总量表的Cronbach′sα 系数为0.82,各分量表的Cronbach′sα 系数为0.78～0.88;总量表的分半信度为0.77,各分量表的分半信度为0.72～0.89;50例患者两次测量的总量表及各分量表得分比较,差异有统计学意义(P<0.001);50例患者两次测量的总量表及各分量表得分间均呈线性正相关(r值为0.53～0.84,P<0.001).344例患者3个分量表得分与总量表得分均呈线性正相关(r值为0.63～0.86,P<0.05),各维度得分与所属分量表得分均呈线性正相关(r值为0.35～0.89,P<0.05).因子分析共提取出16个公因子,对总量表的累计方差贡献率为82.74％,对3个分量表的累计方差贡献率为73.09％ ～76.79％.初治患者和复治患者的BK、BA、BP分量表及总量表得分比较,差异有统计学意义(P<0.05).结论 老年结核病患者自我健康管理评定量表的信效度良好,可作为自我健康管理相关研究的评定工具.%Objective To test the reliability and validity of the Health Self - management Assessment Scale for Elderly Patients with Tuberculosis. Methods We randomly selected 360 elderly patients aged 60 or above with tuberculosis in three communities which were randomly selected from Wuchang District,Hankou District,Hanyang District in Wuhan during September 2015 and September 2016. Then we used the part of health self - management assessment including knowledge (BK), attitude (BA),and behavior (BP)subscales from the Social Intervention Rating Scale for Elderly Patients with Tuberculosis developed by this research team previously to investigate the subjects. A total of 95. 6％ (344 / 360)of the subjects responding the survey effectively were selected as the final subjects. Fifty cases of them were randomly selected and retested two weeks later,and all of them (100. 0％)responded the retest effectively. The reliability for the scale was assessed based on the values of Cronbach′s α,split - half reliability,test - retest reliability,and responsiveness. The validity for the scale was assessed by the values of content validity,construct validity and discriminant validity. Results The value of Cronbach′s α for the scale was 0. 82,and that for the subscales ranged from 0. 78 to 0. 88. The value of split - half reliability for the scale was 0. 77,and it for the subscales ranged from 0. 72 to 0. 89. In the 50 cases who were surveyed for two times,notable differences were found between the two measurement scores of scale,and two measurement scores of subscales (all P < 0. 001);linear positive correlation was found between first as well as second measured scores of the scale and those of each of the subscales (value of r ranged from 0. 53 to 0. 84,P < 0. 001). For the 344 subjects,the scores of each of the three subscales had linear positive correlation with the scores of the scale (value of r ranged from 0. 63 to 0. 86,P < 0. 05);each subscale′s scores presented linear positive correlation with each of its dimensions' scores (value of r ranged from 0. 35 to 0. 89,P < 0. 05). A total of 16 common factors were extracted by factor analysis,which could explain 82. 74％ of the total variance for the scale,and 73. 09％ ～ 76. 79％ of the total variance for the three subscales. Scores of subscales of BK,BA,and BP,and the scale differed significantly between the first -treated and re - treated patients (P < 0. 05). Conclusion The Health Self - management Assessment Scale for Elderly Patients with Tuberculosis demonstrates good reliability and validity,so it can be used as an assessment tool for the studies associated with health self - management.
