舒血宁对急性重度一氧化碳中毒患者心型脂肪酸结合蛋白水平的影响

摘要

目的:探讨舒血宁对急性重度一氧化碳中毒(ASCOP)患者心型脂肪酸结合蛋白(H-FABP)的影响。方法选择河北医科大学附属哈励逊国际和平医院急诊科2012年1月—2015年3月收治的 ASCOP 患者共138例,按照随机数字表法分为治疗组和常规组,各69例。两组患者均在治疗前(中毒后2 h)及治疗后7 h、14 d,检测肌钙蛋白 I(cTnI)、H-FABP、丙二醛(MDA)、血清超氧化物歧化酶(SOD)水平。结果治疗组与常规组总有效率分别为94.2%(65/69)和69.6%(48/69),差异有统计学意义(χ2=14.118,P ﹤0.001)。多因素方差分析显示,时间、治疗方法对两组 cTnI、H-FABP、MDA、SOD 水平的影响均有统计学意义(P ﹤0.05);且时间和治疗方法对两组 cTnI、H-FABP、MDA、SOD 水平的影响有交互作用(P ﹤0.05)。其中,治疗前两组 cTnI、H-FABP、MDA、SOD 水平比较,差异无统计学意义(P ﹥0.05)。治疗后7 h,常规组 cTnI、H-FABP水平较治疗组升高,差异有统计学意义( P ﹤0.05);两组均较组内治疗前升高,差异有统计学意义(P ﹤0.05)。治疗后14 d,两组 cTnI、H-FABP、MDA、SOD 水平均较治疗后7 h 降低,差异有统计学意义(P ﹤0.05)。治疗后14 d,常规组 MDA 水平较治疗组升高,常规组 SOD水平较治疗组降低,差异有统计学意义(P ﹤0.05)。ASCOP 患者病情程度与H-FABP水平呈正相关(rs =0.739,P ﹤0.01)。结论 ASCOP 患者早期给予舒血宁治疗,可改善组织缺氧,清除氧自由基,抗自由基损伤及抑制脂质过氧化,促进器官功能的恢复。%Objective To study the effect of Shuxuening on heart - type fatty acid binding protein(H-FABP)of the patients with acute severe carbon monoxidepoisoning(ASCOP). Methods We enrolled 138 ASCOP patients who were treated in the emergency department of Harrison International Peace Hospital Affiliated to Hebei Medical University from January 2012 to March 2015. The patients were randomly divided into treatment group( n = 69)and convention group( n = 69). Before treatment(2 hours after poisoning),and after 7 hours and 14 days treatment,troponin I(cTnI),H-FABP,malondialdehyde (MDA)and superoxide dismutase(SOD)of patients were detected. Results The total effective rates of the treatment group and the convention group were 94. 2% (65 / 69)and 69. 6% (48 / 69)respectively;the two groups were significantly different (χ2 = 14. 118,P ﹤ 0. 001). Multiple factor variance analysis showed that the influence of time and grouping on cTnI,H-FABP, MDA and SOD levels of the two groups was significant(P ﹤ 0. 05),and the influence of time and grouping on cTnI,H-FABP, MDA and SOD levles of the two groups interacted with each other( P ﹤ 0. 05). Before treatment,the two groups were not significantly different in cTnI,H-FABP,MDA and SOD levels(P ﹥ 0. 05). After 7 hours treatment,the convention group had higher cTnI and H-FABP levels than treatment group(P ﹤ 0. 05);the two groups both had higher cTnI and H-FABP levels than those before treatment(P ﹤ 0. 05). After 14 days treatment ,the two groups had lower cTnI,H-FABP,MDA and SOD levels than those after 7 h treatment(P ﹤ 0. 05). After 14 days treatment,the convention group was higher than the treatment group in MDA level,lower in SOD level(P ﹤ 0. 05). The disease severity of ASCOP patients had positive correlation with H-FABP level ( rs = 0. 739,P ﹤ 0. 01). Conclusion The early administration of Shuxuening on ASCOP patients,can improve tissue hypoxia, eliminate oxygen free radicals,resist free radical injury,inhibit lipid peroxidation and promote the recovery of organ function.
