茶多酚对维持性血液透析患者认知功能和炎性因子的影响

摘要

目的 探讨茶多酚对维持性血液透析(MHD)患者认知功能和炎性因子的影响.方法 60例MHD患者依照完全随机设计方法分为常规组(31例)及茶多酚组(29例),常规组进行规律血液透析机降压等常规治疗,茶多酚组在常规处理基础上口服茶多酚片每次2片,每日3次.3个月后检测常规组及茶多酚组核因子κB(NF-κB)、白介素6(IL-6)和肿瘤坏死因子α(TNF-α)及事件相关电位P300的潜伏期及波幅.结果 与常规组相比,茶多酚组NF-κB[(72.6±19.9)% 比(87.7±23.2)%]、TNF-α[(69±25)mg/L比(88±23)mg/L]、IL-6[(88±22)μg/L比(106±26)μg/L]下降,差异均有统计学意义(P<0.01);P300潜伏期缩短[(365±33)ms比(387±28)ms ]而波幅升高[(11.2±2.3)μV 比(9.7±2.0)μV ],差异有统计学意义(P<0.01).结论 茶多酚可改善MHD患者认知功能,其机制可能与减轻患者微炎性状态有关.%Objective To investigate the effect of tea polyphenols on the cognitive function and inflammatory factors in patients with maintenance hemodialysis ( MHD ) . Methods Sixty MHD patients were according to completely random design method divided as conventional group ( n = 31 ) receiving the conventional treatment by law hemodialysis machine and tea polyphenols group ( n = 29 ) receiving additional tea polyphenol 2 tablets orally. Three months after the parameters of nuclear factor κB ( NF -κB ), interleukin 6 ( IL - 6 ) and tumor necrosis factor alpha ( TNF - α), and event related potential P300 of incubation period and amplitude were respectively measured. Results Compared with conventional group, the levels of NF - κB [ ( 72. 6 ± 19.9)% vs. ( 87.7 ±23.2 )%], TNF-α[ ( 69 ± 25 ) mg/L vs. ( 88 ±23 ) mg/L ], and IL -6 [ ( 88 ± 22 ) μg/L vs. ( 106 ±26 ) μg/L ] were statistically significantly lower ( P <0. 01 ); P300 latency period shorten [ ( 365 ±33 ) ms vs. ( 387 ± 28 ) ms ] and amplitude increase [ ( 11. 2 ± 2. 3 ) μV vs. ( 9. 7 ± 2. 0 ) μV ] ( P < 0. 01 ) . Conclusion Tea polyphenol can improve cognitive function in patients with MHD, its mechanism may be associated with reducing the micro inflammatory state.
