脑卒中患者胆红素水平的性别差异研究

摘要

目的 探讨不同类型脑卒中患者血清胆红素水平变化及性别差异.方法 对136例动脉粥样硬化性脑梗死(以下简称"脑梗死")及102例脑出血患者进行总胆红素(TBIL)、直接胆红素(DBIL)、间接胆红素(IBIL)、尿素氮(BUN)、肌酐(Cr)、尿酸(UA)、空腹血糖(FBS)、总胆固醇(TC)、三酰甘油(TG)检测,并采用整群抽样法抽取同时期在我院进行健康体检者129例作为对照组进行对比分析.结果 脑梗死组TBIL及IBIL水平明显低于对照组,差异有统计学意义(P<0.05);对各组同性别进行比较发现,脑梗死组男性患者TBIL及IBIL水平明显低于对照组男性,差异有统计学意义(P<0.01),而脑梗死组女性患者胆红素和对照组女性相比差异无统计学意义(P>0.05);脑出血组Cr及FBS、脑梗死组FBS水平较对照组显著升高,差异有统计学意义(P<0.05),脑出血组男性或脑梗死组女性患者Cr及FBS水平分别较对照组男性或女性、脑出血组女性患者FBS水平较对照组女性显著升高,差异有统计学意义(P<0.05),而脑梗死组男性患者BUN及Cr水平显著低于脑出血组男性患者,差异有统计学意义(P<0.05).结论 脑梗死男性患者血清TBIL及IBIL处于低水平,为脑梗死的预防提供线索.%Objective To investigate the change and gender difference of serum bilirubin in stroke patients. Methods The serum total bilirubin ( TBIL ), direct bilirubin ( DBIL ), indirect bilirubin ( IBIL ), blood urea nitrogen ( BUN ), creati-nine ( Cr), uric acid ( UA ), fasting blood sugar ( FBS ), triglycerides ( TG ), total cholesterol ( TC ) were measured in 136 atherosclerotic thrombotic cerebral infarction ( hereinafter to be referred to as " cerebral infarction" ) and 102 cerebral hemorrhage patients. And 129 healthy subjects served as the control. Results The TBIL and IBIL levels were significant lower in the cerebral infarction group than in the controls ( P < 0. 05 ). The gender comparison found that the TBIL and IBIL levels were significant lower in the cerebral infarction group than in the controls for males ( P <0. 01 ), but for females there was no statistical difference in bilirubin level between the two groups ( P >0. 05 ). The Cr and FBS levels in the cerebral hemorrhage group, and FBS level in the cerebral infarction group significantly increased as compared with the controls ( P < 0. 05 ); and the Cr and FBS levels for males in the cerebral hemorrhage group or for females in the cerebral infarction group, and the FBS level for females in the cerebral hemorrhage group significantly increased as compared with the controls ( P <0. 05 ). While the BUN and Cr levels for males in the cerebral infarction group were significantly lower than those in the cerebral hemorrhage group ( P <0. 05 ). Conclusion The levels of serum TBIL and IBIL are lower for male patients with cerebral infarction, which gives a clue for prevention of cerebral infarction.
