2型糖尿病肾病患者正五聚蛋白3和超敏C反应蛋白及γ-谷氨酰转移酶的检测及其临床意义

摘要

Objective To investigate the expression and significance of pentraxin - 3 ( PTX3 ), hypersensitive C - reactive protein ( hs - CRP ) and gamma - glutamyltransferase ( GGT ) in patients type 2 diabetes mellitus ( T2DM ) . Methods 131 T2DM inpatients and outpatients admitted to Ningbo First Hospital were divided into three groups according to UAER: normal albuminuria group ( UAER < 30 mg/24 h, 46 cases ), microalbuminuria group ( UAER 30 ~ 300 mg/24 h, 43 cases ) and mac-roalbuminuria group ( UAER > 300 mg/24 h, 42 cases ) . 30 healthy people underwent physical examination in the same period were selected as control group. PTX3 , hs - CRP and GGT of the four groups were detected and compared. Abnormal distribution was expressed by median ( inter - quartile range ), multiple group comparison was performed by using Kruskal - Wallis test and two group comparisons was performed by using Mann - Whitney U test. Results The levels of PTX3 , hs - CRP and GGT among the four groups showed statistically significant differences ( P <0. 01 ) . The PTX3 level in normal albuminuria group was significantly higher than that in control group ( P < 0. 05 ) . The levels of PTX3 , hs - CRP and GGT in normal albuminuria group, microalbuminuria group and macroalbuminuria increased in sequence, and the differences between every two groups were statistically significant ( P <0. 05 ) . Correlation analysis showed that PTX3 , hs - CRP and GGT were all positively correlated with UAER ( r = 0. 667, 0. 633 and 0. 365 respectively, P all < 0. 01 ) . Conclusion The levels of PTX3 , hs - CRP and GGT in T2DM patients were significantly higher than those in healthy people, and their levels will increase with the seriousness of nephropathy. Detection of the three indicators is of good value for the diagnosis of nephropathy in T2DM patients.%目的 探讨2型糖尿病患者血清正五聚蛋白3(PTX3)、超敏C反应蛋白(hs-CRP)及γ-谷氨酰转移酶(GGT)的表达及其临床意义.方法 选取2011年6月-2012年2月宁波市第一医院门诊和住院的2型糖尿病患者131例,根据尿清蛋白排泄率(UAER)将患者分成3组:正常蛋白尿组(UAER<30 mg/24 h,46例)、微量蛋白尿组(UAER 30~300 mg/24 h,43例)和临床蛋白尿组(UAER>300 mg/24 h,42例);另选取同期同院体检健康者30例作为对照组.分别检测并比较4组受试者血清PTX3、hs-CRP、GGT水平.非正态分布资料以[中位数(四分位数间距)]表示,多组间比较采用Kruskal-Wallis检验,两组间比较采用Mann-Whitney U检验.结果 4组受试者血清PTX3、hs-CRP、GGT水平比较,差异均有统计学意义(P<0.01);其中正常蛋白尿组血清PTX3水平高于对照组(P<0.05),正常蛋白尿组、微量蛋白尿组、临床蛋白尿组血清PTX3、hs-CRP、GGT水平依次升高,组间两两比较差异均有统计学意义(P<0.05).相关性分析显示,PTX3、hs-CRP、GGT均与UAER呈正相关(r值分别为0.667、0.633和0.365,P均<0.01).结论 2型糖尿病患者血清PTX3、hs-CRP和GGT水平显著高于健康者,且随着肾损害程度的加重而升高.故检测这3个指标对2型糖尿病患者早期肾损害具有诊断价值.
