奥卡西平治疗新诊断的部分性癫痫的疗效及其对脑电活动的影响

摘要

目的 探讨奥卡西平(OXC)对新诊断的部分性癫痫患者的临床疗效及其对脑电活动的影响.方法 OXC单药治疗新诊断的50例成人部分性癫痫患者,进行开放性自身对照随访研究.观察OXC治疗1、6及12个月时的临床疗效及对脑电活动的影响.结果 OXC治疗1个月时无发作29例(58.0%)、显效9例(18.0%)、有效8例(16.0%)、无效4例(8.0%),总有效率为92.0%;OXC治疗1、6、12个月时疗效比较差异无统计学意义(H=0.559,P>0.05).治疗6个月时间歇期痫样放电(IEA)消失或减少50.0%以上者26例(52%),治疗12个月时IEA消失或减少50.0%以上者29例(58%).治疗6个月时脑电地形图δ、α和β频段相对功率与治疗前相比差异无统计学意义(P>0.05),而θ频段相对功率较治疗前明显增加(P<0.05).结论 OXC单药治疗新诊断的成人部分性癫痫有较好的疗效,并可使痫样放电减少或消失,对脑电背景活动影响较小.%Objective To investigate the curative effect of oxcarbazepine ( OXC ) in newly diagnosed partial epilepsy patients and its impact on brain electrical activity. Methods A self - controlled, open - lable study was carried out among fifty adults with newly diagnosed partial epilepsy receiving OXC monotherapy. Curative effect and impact on brain electrical activity among the subjects were observed after one month, six months, and twelve months of therapy. Results After one month of OXC therapy, the outcome turned out to be complete effective in 29 cases ( 58.0% ); ≥75% seizure frequency reduction was seen in 9 cases ( 18.0% ), ≥50% seizure frequency reduction was seen in 8 cases ( 16.0% ), and ≤49% seizure frequency reduction was seen in 4 cases ( 8.0% ). Total effective rate was 92.0%. No statistical differences in efficacy were observed among 1,6, and 12 months of OXC treatment ( H = 0.559 , P>0.05 ). After six months of OXC therapy, IEA either vanished or showed ≥50% decrease in 26 cases ( 52% ) and the numher was 29 ( 58% ) for twelve months of treatment. As for brain electrical activity mapping, significant increase of the relative power of 0 band was ohserved ( P<0.05 ) while no significant differences of the relative power of α, β and δ bands were noted after six month therapy ( P>0.05 ). Conclusion OXC monotherapy is effective for adults with newly diagnosed partial epilepsy. OXC monotherapy might diminish or even vanish IEA and shows little impact on the hackground of EEG activity.
