目的 探讨斑点追踪显像(speckle tracking imaging,STI)评价原发性高血压(EH)患者早期左心室旋转和扭转运动改变的价值.方法 对35例左心房、左心室正常构型的EH患者(高血压组)和35例健康者(对照组),经胸采集左心室短轴心尖水平和心底水平的二维图像,计算心底水平旋转角度峰值(MV-Prot)、心尖水平旋转角度峰值(AP-Prot)、扭转角度峰值(Ptw).结果 两组受检者心尖部旋转角度均显著大于心底部旋转角度,差异均有统计学意义(P＜0.01);但两组达峰时间比较,差异无统计学意义(P＞0.05);高血压组患者心尖部旋转角度显著大于对照组,差异有统计学意义(P＜0.05).结论 EH患者早期左室整体心肌扭转运动主要表现为逆时针方向扭转,扭转运动增强.%Objective To observe the value of speckle tracking imaging ( STI ) in assessing early changes of left ventricular rotation and twist in patients with essential hypertension ( EH ) . Methods 35 EH patients with normal left atrial and left ventricular and 35 healthy controls were involved into the study. 2 - D images were acquired from the mitral valve and apex of the left ventricular. Peak rotation on basal plane ( MV - Prot ), apical plane ( AP - Prot ) and peak twist ( Ptw ) were measured. Results In the two groups, AP - Prot were significantly greater than MV - Prot ( P < 0. 01 ), but the peak time between the two groups was not significantly different ( P > 0. 05 ) . AP - Prot in EH group was significantly greater than that in control group ( P < 0. 05 ) . Conclusion The motion character of early left ventricular myocardium of EH is mainly counter - clockwise twisting, and Ptw is increased.
