目的 观察小剂量重组人促红细胞生成素(rhEPO)应用于早期慢性肾衰竭(CRF)患者时,对其左心功能的影响.方法 选取我科2008年2月-2010年6月收治的58例非透析的CRF患者,随机分为两组,治疗组32例采用小剂量rhEPO治疗,对照组26例不予注射rhEPO,两组均控制血压于正常水平,并给予低蛋白饮食及口服降肌酐药物治疗.6个月后观察并比较两组患者血红蛋白(Hb)、血清B型尿钠肽(BNP)水平、血清肌酐清除率(Ccr)及心脏彩超检查等情况.结果 6个月治疗结束后,两组干预后各项检查指标分别与干预前比较,差异均无统计学意义(P＞0.05).治疗组干预前各项检查指标分别与对照组比较,差异均无统计学意义(P＞0.05).治疗组干预后Hb、心脏指数(CI)、Ccr分别与对照组比较,差异均无统计学意义(P＞0.05);而治疗组干预后BNP水平、Devereux校正的左心室重量指数(LVMI)、左房室瓣部位舒张早期和晚期最大血流速度比(E/A)、E峰加速度(Eacc)分别与对照组比较,差异均有统计学意义(P＜0.05).结论 小剂量rhEPO应用于早期CRF患者可改善其左心功能.%Objective To investigate the effect of recombinant human erythropoietin ( rhEPO ) on left cardiac function in patients with chronic renal failure ( CRF ) . Methods Fifty eight non dialysis patients with CRF admitted to our hospital from February 2008 to June 2010 were randomly divided into treatment group and control group. In treatment group, the 32 patients were given rhEPO. In control group, the 26 patients were not given rhEPO. Blood pressure of all patients in the two groups was controlled at the normal level during the treatment. All patients were given low protein diet and medicines for reducing creati-nine level. The levels of Hb, BNP, Ccr and color ultrasonic test of the two groups were observed and compared after six months' treatment. Results After six months'treatment, each index of the two groups showed no significant difference compared with those before intervention ( P > 0. 05 ) . Each index of the treatment group before intervention showed no significant difference compared with control group ( P > 0. 05 ) . After intervention, Hb, CI and Ccr of the treatment group showed no significant difference compared with control group ( P >0. 05 ) . After intervention, the level of BNP, left ventricular mass index ( LV-MI ), left atrioventricular valve area of early and late diastolic blood flow velocity than the maximum ( E/A ) and E peak acceleration ( Eacc ) showed significant difference compared with control group ( P < 0. 05 ) . Conclusion Small dose of rhEPO could improve left cardiac function in the treatment of patients with chronic renal failure.
