目的：通过与冠状动脉造影结果对比，评价平板运动试验对冠心病的诊断价值及相关因素分析。方法：对平板运动试验阳性的100例患者的资料进行回顾性分析。与同期的冠状动脉造影结果相比较，分析平板运动试验的真阳性率及假阳性率及其与年龄、性别的相关性；分析冠状动脉病变支数与试验阳性率的相关性。结果：运动平板试验的真阳性率为71％，假阳性率为29％；在＜50岁、50～60岁、＞60岁患者中，男性真阳性率依次为90％、75％与50％，女性依次为20％、68．8％与87％，男女不同的年龄组间均有显著的统计学差异（ P＝0．0333与 P＝0．0047）；冠状动脉造影结果病变阳性支数为1、2、3支的构成比依次为40％、23％与8％，单支病变男女比例分别为27．5％与48．3％，而双支及多支病变男女分别为32．5％与16．7％及10．0％与6．7％。男女性别差异有统计学意义（ P＝0．008）。结论：平板运动试验真阳性率在小于50岁的男性中较高，要引起重视；在小于50岁的女性中假阳性率高，诊断冠心病要慎重；平板运动试验真阳性患者中，男性的冠状动脉病变受累血管多于女性。%Objective:To evaluate the diagnostic value and influence factors of treadmill test for coronary heart disease by comparing with the results of coronary angiography .Methods:A total of 100 patients with positive treadmill test results who underwent coronary angiography were retrospectively analyzed .The correlation between the positive rate and the age ,gender and the number of lesion vessels was evaluated .Results:The true‐positive rate and false‐positive rate of treadmill test were 71% and 29% .True‐positive rate was 90% ,75% ,and 50% in males;whereas 20% ,68 .8% ,and 87% in females across three age groups (under 50 years old ,50‐60 years old ,over 60 years old) ,indicating a P value of 0 .033 3 in males and 0 .004 7 in females .The true‐positive rates with single ,double ,and triple vessel diseases were 40% ,23% ,and 8% ,respectively .In single‐gender analysis ,it specifies 27 .5% ,32 .5% ,and 10 .0% in males ,while 48 .3% ,16 .7% ,and 6 .7% in females .This results indicated a significant statistical difference between males and females in the number of vessel diseases (P= 0 .008) . Conclusions:The true‐positive rate was high in males under 50 years old ,while the false‐positive rate was high in females under 50 years old .This analysis has suggested that in those who had yielded positive treadmill test results ,males were more likely to demonstrate a larger number in vessel diseases than females did .
